Nursing Home Transition Leaving the Facility. Coming Home.
DON Services provides nursing home transition in Western PA, helping people with disabilities leave nursing facilities and move back into home and community settings. Our Independent Living Coordinators handle the coordination — housing, community resources, and discharge planning — across all four DON Services offices.
Who Qualifies for Nursing Home Transition You may be a good fit for this program if:
Nursing Home Transition is coordinated through Pennsylvania’s Office of Long-Term Living, which oversees the state’s nursing facility transition process.
A transition can be started by you, a family member or power of attorney, or the nursing facility’s discharge planner. However it starts, DON becomes your point of contact and does the coordination work on your behalf — you don’t have to navigate it alone.
